Excavation was backfilled at the end of work day. 5/04/21 2:00pm Tuesday Contractor (Beemer Ben’s Crew) installed 5’X3 curb inlet and 24” HDPE corrugated pipping see page C 3.3. 5/05/21 Wednesday 12:35pm Wednesday Contractor (Beemer Ben’s Crew) installed 5”X3” curb inlet and the (N-12) 24” HDPE see page C 3.3. 3:25pm Contractor continued installing 60’ of corrugated (N-12) 18” HDPE . 5/06/21 Thursday Contractor (Beemer Ben’s Crew) Installed approximately 80’ of 18” N12 HDPE corrugated storm piping. Progress slowed because contractor began excavation of rock on project. 05/07/21 Friday Contractor (Beemer) installed approximately 115’ of 18” HDPE corrugated storm piping from Sta. 1+85 to curb inlet (5’X3’) 1-3 . Contractor began installation of 15” HDPE corrugated storm piping from curb inlet 1-3 to Sta. 3+20 Storm Line 1. 05/10/21 Monday Contractor (Beemer ) installed approximately 100’ of 15” HDPE Corrugated storm on Line 1 and set curb inlet (5’X3’) 1-4. Condition were muddy and the contractor had to break ground in heavy rock area. 05/11/21 Tuesday Contractor (Beemer) Installed approximately 88’ of 24” HDPE of corrugated Storm pipe on Line 2 and set 2-3 Curb Inlet (5’X3’). Contractor continued the installation of Line 2 which goes to 18” HDPE Corrugated piping. 05/12/21 Wednesday Contractor (Beemer) 60’ of 18” HDPE of corrugated storm pipe. was installed yesterday and 20’ today. 2-4 Curb Inlet (5’X3’) was 10:00am. 05/13/21 Thursday Contractor (Beemer) will be pouring inverts and collars on storm line 2. 05/24/21 Monday Contractor (Beemer) will continue to pour inverts & collars on line 2 and a collar on line 1. 05/25/21 Tuesday Contractor performed general contracting duties on site like backfilling, placing lids on CI, and offloading excavation equipment. 05/26/21 Wednesday Contractor was able to complete hydrostatic on fire line at 200 psi for 2 hours. Contractor also worked on grease trap vault that should be inspected by codes. 05/27/21 Thursday No work due to rain. 05/28/21 Friday Contractor replace Rip Rap that was silt over due to the rains also general dressing of site to repair soil erosion on site.

4/29/21 Thursday Contractor (Beemer was not on worksite) water tap and meter was drop off @ site today. Contractor (Ben Foreman Beemer) called to see if water had done a test shut of 12” water main. I informed the contractor due to a heavy work load was able to get to the test shut. Note: (I talk with Beemer Ben Foeman & Chad Project Supervisor about informing inspector when contractor return back to work on a project that has had no work activity for an extended period. Delay can happen when pre-planning is not done). 4/30/21 Friday Contractor (Beemer Ben’s Crew) working install of meter and Copper (K”) as per city specifications. 1:30pm contractor has set the meter and ran “K” pipe and is installing pipelines that run to and around Backflow Preventer. 5/3/21 Monday Contractor (Beemer Ben’s Crew) install fire hydrant bends fittings & valve as pre plan for fire line. 2:14pm Contractor poured backing blocks behind bends & fittings and continued install Water Main as per plan. 5/4/21 Tuesday Contractor (Beemer Ben’s Crew) continued to install water line to the proposed building. Inspector met with Water Utilities at site to plan for tie-in as per plan. As of today we trying to track down all valves to shut down 12” Water Main . It appears that the Subaru Auto dealership will be effect by shut. Once all valves are located a plan will be put in place for the tie-in. 5/13/21 Thursday Contractor install tie-in on on 12”X6” tee on 12’’ water main. Contractor is. Pouring backing block and gravity block with restraint and steel reinforcements on 22 deg bend. Tie was completed at 11:20 am .

The contractor had to modify encasement to get the necessary clearance from exist manhole. 6/11/21 Friday Contractor removed steel encasement and reassembled the encasement protective covering. Contractor is trying to get approval to tap 12” sanitary main. 6/14/21 Monday Contractor Beemer had to cut a foot more of encasement to install Wye and Hard back Fernco couplings on 12” sanitary main. 6/15/21 Tuesday Contractor Beemer continues to install force main for building project with Locate (see pic & video.) Storage or septic Tank was installed to grade as per contractors plans. 6/17/21 Thursday Contractor Beemer backfilled around the the manhole area and installed check valve (on out flow side stops waste from returning to the septic tank) and will connect 4” sanitary main to Septic tank in flow side. The detention basin is not functioning properly on this site. We have talked with Ryan and Dan and come to the agreement that the developer will be able to put up a line of credit to the City for the detention basin. In event that the contractor does not complete the basin and have it functioning properly we will have the line of credit to finish the job. We will start out with a 30 day TCO and will renew it at that point if we are seeing progress being made. If progress is not being made we will revoke the TCO and use the court process. Adam and I will be on-site at 1:30 to check in with you. Needed for TCO -Approved amount for the line of credit for the detention basin based on the engineers estimated cost. -Approval that we have received the line of credit. Needed for Final Occupancy -Approved re-design of the detention basin -Approved As-graded and as-builts of the detention basin when completed -Establish 70% vegetation on the area’s that have been disturbed and in the basin. Nothing has been done to stop sediment from entering the basin. Still no approved plans for correcting the basin. Their TCO is about to expire I believe around July 15th.

7-14-22 AN fine cut is out and cutting the storm box down and they are using the approved epoxy for the rebar. When the regrading is being done the basin also needs to be regraded and the riser pipe also needs to be cleaned out. We will also need as graded and as builts 7-15-22 RH. 5-2-22 AN pics attached. Need to unclog overflow pipe and get silt fence up or rock checks installed. Excessive amount of silt built up. 7-7-22 RH. Nothing has been done to stop sediment from entering the basin. Still no approved plans for correcting the basin. Their TCO is about to expire I believe around July 15th.

7-14-22 AN fine cut is out and cutting the storm box down and they are using the approved epoxy for the rebar. When the regrading is being done the basin also needs to be regraded and the riser pipe also needs to be cleaned out. We will also need as graded and as builts 7-15-22 RH. Should be a crew out next week doing rest of the grading. 7-18-22 AN no work on the basin today 7-19-22 AN no work on the basin today 7-20-22 AN no work on the basin today 7-21-22 AN no work on the basin today 7-26-22 AN survey stacks are in place so they can start grading 7-27 to 28-22 AN. Its been raining and no work being done 8-1-22 AN Mid state and began to grade the basin 8-2-22 AN grading has been done on the dam but the basin still needs the silt cleaned out. Riser pipe cleaned out , as graded , as builts 8-3-22 AN no work on basin 8-4-22 AN no work on basin 8-8-22 AN met terry and at the site and let him know that the silt needs to be cleaned out, re vegetation and also needed as graded and as builts.
