04/06/22 Wednesday (Star Development - Aaron (816) 807-2502) Silt fence has failed throughout the project. Silt fence needs to be re-staked and repaired. 04/18/22 Monday (Star Development- Aaron) I called contractor and informed him that the erosion control had failed. He expected the erosion fencing company to be out in the next few days. I will check site again to follow up progress on re-establishing erosion control measures. 04/21/22 Thursday (Star Development-Aaron) I again called Aaron about the failed silt fencing around the project. Aaron said he was going to come to the site today with the silt fencing company and re-establish the erosion control. 04/22/22 Friday (Star Development-Aaron) I called Aaron to let him know that there will be No Work on site until erosion control measure are in place. 04/28/22 Thursday Erosion control silt fence was corrected 04/27/22 Wednesday. Contractor resume work on site.

Contractor will be install approximately 200’ of 15” Storm as per plan. 05/05/22 Thursday (Sub Contractor For Star Development) Contractor began work on 5/3/22 Tuesday. On 05/09/22 Monday I will meet with contractor and review all storm sewer construction on site. 05/09/22 Monday Contractor continued storm installation storm box 1,2, & 3 Were set early last week. The last box should be set by tomorrow. Approximately 160’ of 15” Storm pipe HPDE was installed, also last week between the storm boxes. 5/20/22 Friday Contractor laid approximately 70’ (15” green sanitary pipe)the storm pipe from 3 storm box to 4 storm box. Storm box 4 was set last week. See pics of storm line.

This will allow a tapping point for the domestic and irrigation water main as-per the City of Lee’s Summit. This change will be reflected on the as built.
