On Monday 4-25-2022 Amino Brothers Backfilled between the east of the east entrance towards the first CI for the street lane widening. On Wednesday, 4-27-2022, the contractor is digging unsuitable fill east of the east entrance down to the CI’s and is installing geo-grid and shut rock. The contractor is also placing shot rock west of the east entrance where the fill was not completed prior. Approx 150LF. On Thursday. 4-28-2022 Amino brothers resume rock installation for the lane widening at the east side of the east entrance. Rain has come in and is expected off and on for the day. On Friday, 4-29-2022 Amino Brothers resume rock sub-base installation at the east entrance.

Amino Brothers begin removals of curbing and fill dirt where fiber cables were located at the SE corner of Bailey and Hamblen. AT&T have completed the fiber phone line relocation.

On Tuesday, 4-05-2022, The Amino Brothers Construction Co. installed CI box E-1 at the east end of the entrance. It was found the honch that was designed to go around the sides of the box came without them. The contractor was notified and I will ask for the solution from an engineer. The contractor also completed demo of the curbs along the front of school. On Wednesday, 4-06-2022. Amino Brothers installed the 12’x4’ CI and approx 40 ft 15” of RCP. The 2 - 36” RCP pipes that tie into the existing CI box are squid to the right and will not meet flush into the existing CMP pipe. On Thursday, 4-07-2022. The contractor installed CI E-3 and approx 15 ft of RCP. On Friday, 4-08-2022 it was discovered, The easterly 36” RCP pipe will drain towards the center wall of the new box and the front wall of the new box has been removed due the askew of the pipe. The contractor has poured the invert and front wall back before I could I could notify the engineers of the findings. Ryan Fleming of Olson has been notified of the findings.

On Monday, 5-02-2022. The Amino Brothers began installation of the haunches on the east and west sides of the prior installed CI’s located east of the east entrance of the school. It was also determined. The existing CI that is supposed to be made a 10x4 junction box is actual (2) 5x4 CI’s. After talking with the designer Ryan Fleming of Olson Engineering. It was determined we are going to change purposed CI junction box to a inclosed storm sewer system by modifying the existing boxes and filling them in with concrete. This will allow us to eliminate the lid and cover the structure with asphalt.

On Friday, 4-15-2022. It was determined the sub-grade for the lane additions in front of the middle school were to wet to stabilize. The 3rd party testing firm suggested the contractor Amino Brothers place multiple layers of 3” shot rock with geo-grid for a total of 10” west of the west entrance and 14” of rock in 6” layers with geo-grid east of the west entrance. Then a proof roll will be performed. The contractor completed the west roadway widening.

Found a curb elevation bust at the west entrance on the east side of the drive entrance. Approx 20’ of curb and gutter will have to be removed and replaced to meet ADA spec’s. Amino Brothers set all (3) CI’s on Century and Bailey and poured inverts. The contractor in countered a AT&T phone line with the far north box at the Day care. The contractor dug around the phone line an got slack, set 6” of rock for CI to sit on top of the phone line . I took pictures and will discuss with others.

Amino Brothers formed and poured approx 200 LF of 10’ wide sidewalk along the far west end of the west entrance at the school and at the ADA ramps at the east entrance, west and east ramps. Contractor formed and poured approx 100’ of 10’ wide city sidewalk at the Far East end of the line tapper at the east end of the project. On Tuesday, 7-19-2022. Amino Brothers remove curbing poured pervious at the east side of the west entrance and re-formed to meet the designed profile for that side of the ramp for that entrance. Also contractor poured east ramp for east entrance and also poured approx 80’ of 10’ wide sidewalk at the CI’s and the end of the lane merge On Thursday, 7-21-2022. Contractor formed curb formed removed day prior at the NE corner of Bailey and Century. On Friday 7-22-2022. Amino Brothers poured ADA ramps at the SE and SW corners of the intersection of Bailey and Hamblen. Contractor also poured CG-1 curb at the NE corner of the intersection, pouring to the Child Prodigy school.

On Monday, 8-01-2022. Jeff with McCown Gordon called me and requested a proof roll at Bailey and Ranson Rd. Contractor knows this is the states property and not Lee’s Summits jurisdiction. I witnessed a proof roll for the turn lane on Ranson west side of street heading south bound. Found (3) small areas that contractor will place 18” of 3” shot rock and cap with geo-grid and type 5. Pictures will be taken showing rock placement. Contractor was made aware I will not pass or fail sub-grade. I am observering sub-grades condition. On Monday and Tuesday 8-02-2022. Teague Electric is pouring all concrete caps for pull boxes and bases. On Wednesday, 8-03-2022. Amino Brothers Formed and poured (2) ADA Corners. One at 15th & Dalton, with approx 50 LF of 4’ wide sidewalk. At 15th & Cape the contractor poured 5’ ramp but should have poured a 6’ one Contractor will replace ramp to designed size. Approx 25 LF of 5’ sidewalk was poured at that location as well. The Amino Brothers formed 10’ wide sidewalk at the lane widening between Broadway and Hamblen on Bailey and poured approx 90’. On Friday 2-11-2022. Teague Electric set steel and bolts for concrete placement for the NE SE & NW corners . The Traffic electrical box footing is also poured. The goggle fiber group is on site relocating the fiber lines on the SE corner due to a location adjustment. On Monday, 2-14-2022. The contractor resumed signal base installation on the SE corner of the intersection. Contractor drilled base, set steel cage and poured base. Contractor also did some conduit boring for the traffic box.

On Wednesday, 8-10-2022 Amino Brothers cleaned sidewalk and punch list items given yesterday. Teague Elec. turned on signal lights at Bailey and Century however lights were placed back at blinking red by traffic due to incomplete work at the intersection. On Tuesday, 8-16-2022. Amino Brothers formed sidewalk at the south side of street from 60 ft east of Hamblen and Bailey to the full length of the block at the animal shelter. Contractor ran out of forms to continue. Contractor was told that sub grade for a lot of the sidewalk formed would not meet compliance to place concrete. Rock removal and re-grading would be needed to pour concrete tomorrow. On Wednesday, 8-17-2022. Amino Brothers formed and poured back sidewalk at various locations on Bailey from Hamblen to Norwood. They also poured both commercial drives at the day care and across the street for KC Dumpsters. Finally the contractor placed asphalt for the SE corner of Bailey and Hamblen and at the drive entrance of KC Dumpster. On Thursday, 8-18-2022, Contractor formed and poured ADA Ramps at the intersection of Century and Bailey, at the west corner of Norwood and Bailey and finally the ramp at the east corner of Broadway and Bailey.

Pt Sunny 73 degrees On Tuesday, 3-15-2022 through Thursday 3-17-2022. Amino Brothers completed dirt removal for lane widening between Hamblen and SE Norwood on Bailey. Contractor began placement of Geo-grid and rock for approx 300 LF of lane. Contractor cannot complete the total lane fill dirt removal due to a utility pending relocation.
