There is water with silt leaving the site at the bottom of Main St. Main St. Is covered in dirt and debris and it is raining lightly. I have left a voice and text message with the superintendent of the site about the erosion issues.

I informed the superintendent that we require KCMMB 4K mix for the curb and it must be cured white.

They are waiting on trucks for the east side along Douglas. I spoke with the Forman for Freedom about curing the back of curb and grouting any honeycomb when they pull the forms. 12:35pm I met with Cityscape to go over the saw cuts for the asphalt. 2:37pm They are done pouring the curb. They pulled forms so I had them cure the back side of the curb.

Cityscape will be placing them today. 6-16-21 10am There are no erosion control issues at this time. 6-21-21 8:43am 66 degrees and overcast. The gutter buddy’s on 2nd street are full of silt from runoff of Main St. I spoke with Scott the superintendent and he said he would address the issue. They do not have the straw waddles in place on Main St. And he said he would fix that as well. The rest of the site seems to be good. 7-9-21 8:15am Freedom Concrete are setting up the sidewalk along Douglas and 2nd St. We discuss the ADA at the corner of 2nd and Douglas. They are aware that they need a ADA compliant 5x5 turn space at the corner. They have plenty of aggregate and it is compacted. 4:11pm They have poured the sidewalk and cured with white. The signal pedestal is in the middle of the walk. They will need to pour a minimum of 4’feet on one side of the pedestal. The walk will need widened by the storm inlet by one foot as well. I have talked to Jason with Cityscape about what needs to be done.

6-2-21 10:49am Breit is on site digging up the water main on Douglas to install a new line of copper to the meter pit. The meter pit is being relocated 7’ to the south. 6-3-21 The meter pit is set to spec and the line has been ran. They will be using flowable fill and concrete for the patch on Douglas. 6-3-21 2:46pm Cityscape did not have the correct signs in place for there road closure. I spoke with the superintendent and had them open the lane back up. The lane will stay open until the correct signs are in place. They will be pouring curb on Douglas and 2nd street on 6-4-21. I informed the superintendent that we require KCMMB 4K mix for the curb and it must be cured white. 6-9-21 2:03pm I spoke with Cityscape about adding additional signs on 2nd Street. 7-9-21 8:15am Freedom Concrete are setting up the sidewalk along Douglas and 2nd St. We discuss the ADA at the corner of 2nd and Douglas. They are aware that they need a ADA compliant 5x5 turn space at the corner. They have plenty of aggregate and it is compacted. 4:11pm They have poured the sidewalk and cured with white. The signal pedestal is in the middle of the walk. They will need to pour a minimum of 4’feet on one side of the pedestal. The walk will need widened by the storm inlet by one foot as well. I have talked to Jason with Cityscape about what needs to be done. 7-26-21 3:20pm I met Jason on site to inspect the sidewalk. They need to add the aggregate then they will be ready to pour. 7-27-21 9:52am They have the rock in and will be pouring tomorrow. 7-28-21 1:03pm They have poured the sidewalk on the corner of Douglas and 2nd street. 7-28-21 2:20pm The 5x5 turn space at the corner of Douglas and 2nd is not ADA compliant. I had Matt Munger and Bob Burns come out with their levels to confirm. Jason was on site and became ill with the heat. Because of existing utilities (pedestrians traffic control signal set in the middle of sidewalk & metal plate of utility box) allowances were made to match existing conditions. ADA, adjacent sidewalk & curbs will be constructed to standards. 8/19/21 Thursday Met with contractor Cityscape Jason Colbern onsite with LS Rightway Supervisor Bob Burns, Joe Starlin Public Works CIP Inspector ADA, adjacent sidewalk & curbs contractor has sufficient met the standards given the circumstances of the road & existing utilities. 9/24/21 Friday Contractor didn’t contact me to pre-inspect forms for sidewalk & curbs! I met with Jason of Cityscape and over 70’ of sidewalk and 25’ curb must be remove @ 2nd Street & Main St. Two ADA Ramp were out of compliance. The sidewalk contractor (Summit Concrete -Pedro) will call me when forms are set for re-inspection. Supervisor has been notified of the current issues. 9/28/21 Tuesday Contractor removed sidewalks & curbs that didn’t met City Design Construction Manual requirements. Contractor will use blankets to cover sidewalks due to incoming weather conditions. 04/08/22 Friday Contractor began to excavate area for sidewalks that will be the landing for stairway. 04/11/22 Monday ( Cityscape) Contractor has pour the stairs and the land of the sidewalk on Douglas ST , North of SE 2nd ST. The contractor will install 3 dowels and expansion joints to tie-in the curbs that were done earlier in the project. 4/13/22 Wednesday (Cityscape) During inspection yesterday the issue came up about the placement of storm/drain pipes undermining the retaining wall. The contractor also did not notify the inspector that storm piping was being installed. The contractor has said they had their structural engineer inspect the area in question. He will be sending out his approval and detailed information of what was installed, location, and why it is functional. 05/06/22 Friday (Cityscape) Contractor framed sidewalks, materials and forms were Pre-inspected on 05/05/22 Thursday. Contractor schedule to pour & cover sidewalks after pouring due to possible light showers. 06/09/22 Thursday Contractor continued to pour sidewalks on private/commercial section of property. 06/30/22 Thursday Contractor formed sidewalk to pour approximately 80’ along Douglas Street & 1st Street. 07/01/22 Friday Contractor was able to pour the sidewalk along Douglas Street around 10:30AM. During the forms check for driveway, sidewalk & existing sidewalk and driveway on 1st Street. The contractor did not get an inspection of ADA ramp or turn space which was all out of compliance. 6-2-21 10:49am Breit is on site digging up the water main on Douglas to install a new line of copper to the meter pit. The meter pit is being relocated 7’ to the south. 6-3-21 The meter pit is set to spec and the line has been ran. They will be using flowable fill and concrete for the patch on Douglas. 6-3-21 2:46pm Cityscape did not have the correct signs in place for there road closure. I spoke with the superintendent and had them open the lane back up. The lane will stay open until the correct signs are in place. They will be pouring curb on Douglas and 2nd street on 6-4-21. I informed the superintendent that we require KCMMB 4K mix for the curb and it must be cured white. 6-9-21 2:03pm I spoke with Cityscape about adding additional signs on 2nd Street. Citizens are getting confused or are not paying attention and driving into on coming traffic. If additional signs don’t correct the issue they will need a flagger. They are addressing the issues right away. 6-14-21 11:01am I spoke with Cityscape about replacing the straw waddles at Main and 2nd. There are no inspections needed at this time. 6-17-21 Bob Burns and I met with the superintendent about traffic control issues. They have had trouble previously at the intersection of Douglas and 2nd. They said they are aware and will have the repairs done by the end of next week. They have patched the road. 10-12-21 Tuesday (Total Electric) Contractor installed 2 control pedestrian boxes for the intersection of SE Main Street & SE 2nd Street. 11/04/21 Thursday Site visit: contractor stripped road, poured sidewalks and continued grading site. 02/04/22 Friday Contractor is not working on sidewalks due to weather conditions. 02/07/22 Monday No activity on the sidewalk of project due to weather conditions. Snow is still melting. 02/22/22 Tuesday No work on sidewalks due to weather. 02/23/22 Wednesday No work on sidewalk due to weather. 02/24/22 Thursday No work. 03/16/22 Wednesday Not performing work that required inspection by Lee’s Summit Development Department. Traffic control measure were in place as per project design and instructions. 06/21/22 Tuesday I pass on the information to Cityscape that they had to jet the lines they had installed due to debris in storm line. I emailed Ryan McCormick that contractor was aware that the lines need to be cleared. 07/21/22 Thursday JMJ ( Cityscape/Wil-Pav ) Tom Taylor and I met with contractors to go over scope of repairs of street and installation of parking lot and driveway, sidewalk @ 11:00AM. Contractor will begin on Monday with repairs/rehab and installation. 07/25/22 Monday JMJ ( Wil-Pav ) Contractor began milling 1st Street. Inspected the project for the upcoming TCO on Wednesday 07/27/22 for the Northside of project. 08/05/22 Friday JMJ Wil-Pav complete approximately 5’x8’ asphalt wedge along the newly installed outdoor approach parking lot area. 5’x10’ asphalt patch made to repair newly overlay street on 1st street near the entrance of underground parking lot. Sidewalk repairs on the Eastside of Main Street.

10/26/21 Tuesday (Wil-Pav Co.) Contractor laying Type 5V for surface coat over milled street area of SE Main Street. Contractor started asphalting around 3:45PM. Surface was tacked before asphalt was laid. Approximately 100 tons of asphalt will be installed today on SE Main Street.

5/27/22 Friday ( Wil - Pav ) Contractor is laying asphalt along the Westside of Douglas St where new sidewalks and curbs have been installed for the newly constructed Downtown Apartment. Approximately 340’ length and 2.5’ wide will be installed. Contractor ran out of asphalt and will comeback next week to finish an approximate 35’ length. 06/17/22 (Wil-Pav) On 06/15/22 Wil-Pav returned and completed the 35’ of asphalt that was not done on 05/27/22 and an additional 40 plus around newly installed through for storm box on Douglas Street.

6-2-21 10:49am Breit is on site digging up the water main on Douglas to install a new line of copper to the meter pit. The meter pit is being relocated 7’ to the south. 6-3-21 The meter pit is set to spec and the line has been ran. They will be using flowable fill and concrete for the patch on Douglas. 6-3-21 2:46pm Cityscape did not have the correct signs in place for there road closure. I spoke with the superintendent and had them open the lane back up. The lane will stay open until the correct signs are in place. They will be pouring curb on Douglas and 2nd street on 6-4-21. I informed the superintendent that we require KCMMB 4K mix for the curb and it must be cured white. 6-9-21 2:03pm I spoke with Cityscape about adding additional signs on 2nd Street. Citizens are getting confused or are not paying attention and driving into on coming traffic. If additional signs don’t correct the issue they will need a flagger. They are addressing the issues right away. 6-14-21 11:01am I spoke with Cityscape about replacing the straw waddles at Main and 2nd. There are no inspections needed at this time. 6-17-21 Bob Burns and I met with the superintendent about traffic control issues. We informed them to have proper flaggers for any traffic situations. 6-22-21 11:35am There are no inspections needed at this time. The site is clean at this time. 6-23-21 There are no inspections needed at this time. The site is clean. 7-6-21 8:19am Cityscape have a lane drop on Douglas and on 2nd St. I spoke with Jason Colbert with Cityscape about getting the proper signs in place. They do not have a “No left turn” sign on Douglas for people trying to go west on 2nd. They also need a “must turn left” sign on 2nd St. directing cars onto Douglas that are heading west. They have had trouble previously at the intersection of Douglas and 2nd. With people being confused. 7-8-21 11:30am Dawn Bell, Steve Robbins and I met with Scott the superintendent for Cityscape on site to discuss the fiber line that has been hit in two places. They said they are aware and will have the repairs done by the end of next week. They understand that this was caused by their construction. 7-12-21 4:03pm There were no inspections needed from me today. 7-13-21 9am Dawn Bell, Bob Burns, Erin and I met on site with the superintendent Scott about lane closures. Cityscape is submitting a traffic control plan. 7-14-21 9:25am The site is picked up and there are no inspections needed at this time. 7-20-21 9:17am They have not removed the materials blocking the manholes on 1st. 7-9-21 8:15am Freedom Concrete are setting up the sidewalk along Douglas and 2nd St. We discuss the ADA at the corner of 2nd and Douglas. They are aware that they need a ADA compliant 5x5 turn space at the corner. They have plenty of aggregate and it is compacted. 4:11pm They have poured the sidewalk and cured with white. The signal pedestal is in the middle of the walk. They will need to pour a minimum of 4’feet on one side of the pedestal. The walk will need widened by the storm inlet by one foot as well. I have talked to Jason with Cityscape about what needs to be done. 7-26-21 3:20pm I met Jason on site to inspect the sidewalk. They need to add the aggregate then they will be ready to pour. 7-27-21 9:52am They have the rock in and will be pouring tomorrow. 7-28-21 1:03pm They have poured the sidewalk on the corner of Douglas and 2nd street. 7-28-21 2:20pm The 5x5 turn space at the corner of Douglas and 2nd is not ADA compliant. I had Matt Munger and Bob Burns come out with their levels to confirm. Jason was on site and became ill with the heat. We all agreed that four panels need to be removed to make the ADA work. 8/17/21 Tuesday Met with contractor Cityscape (Scott Brunell), LS Rightway Supervisor Bob Burns, & Matt Munger Rightway Inspector about ADA & sidewalk. Because of existing utilities (pedestrians traffic control signal set in the middle of sidewalk & metal plate of utility box) allowances were made to match existing conditions. ADA, adjacent sidewalk & curbs will be constructed to standards.
