Matt Munger 02/14/2018 9:00 AM John D. and I looked over the water materials that were on site. There was approximately 630 Linear Feet of C900 PC 235 on site. The fittings and tees had the fusion bonded epoxy coating on them. 3M Rubberized coating spray for the bolts and there was Class 52 DI pipe on site. All materials were in good condition.

Gene stated the contractor can use C900 to bore under the road to avoid doing steel pipe casing. Spoke with Rick on site and informed him to use the C900 per Gene Williams and make sure they use tracer wire.

Matt Munger 03/15/2018 2:11 PM John D. and I observed Winwater perform the wet tap on the 16" water main. Winwater pressure tested the connection. I sent Ken Parker the detail for water thrust blocks and fittings from the Design and Criteria Manual. The contractor made sure to use 3M Rubberized spray on the bolts and to use the clear poly wrap material around the fittings and 16" water main. I informed Ken we will need to perform a pressure test, chlorinate the line and 24 hours or more before that we would need to contact the Water Department to come out and install a construction tap, then after all necessary tests pass Water would issue a stainless steel repair clamp that Ken would be required to install. Note: Ken said they would not be backfilling today.

There appears to be sufficient rock bedding under the pipe. The thrust block appear to meet the City of Lee's Summit design criteria. The fittings used are approved according to the D&C.

John DiMauro 11/30/2018 9:24 AM On Tuesday 11/27/18 The city of Lee's Summit Water Utilities department performed a camera inspection on the public section of the sanitary sewer line. Water Utilities identified defects with the line and the line appears to be active. They identified that the service line is entering into the sewer main below the flow line and coming up into the pipe. Since the wye is coming up into the main, the service line is holding lots of waste and not properly draining into the main. Another issue is the main that enters into the existing manhole is holding a lot of water. Please make the proper corrections to the sanitary service line.

6/19/2018 The WilPav crew was on site installing the remainder of the base asphalt lift. I verified asphalt type, temperature and thickness. During the afternoon a section of the base lift was rained on during installation. It was approximately one truck load on the southwest area of the drive into the parking lot. Rick Mullen, the property owner, is aware of the issue and is going to address it with WILPAV.

Matt Munger 02/16/2018 11:42 AM Spoke with the boring contractor and asked how he was going to get the poly wrap around the DI pipe, I contacted Gene in regards and he stated the pipe should be encased since DI pipe needs to be wrapped with polywrap. Gene stated the contractor can use C900 to bore under the road to avoid doing steel pipe casing. Spoke with Rick on site and informed him to use the C900 per Gene Williams and make sure they use tracer wire.
