Jeremy Young 03/01/2018 1:19 PM The sub-contractor Max Reike Co. Installed the 10" Wye connection into the existing sanitary main. The Wye connection had 2' of pipe on each side and was connected with Strongback Fernco fittings. The Wye had a 4" connection and was installed between 30-45%. The line had 6" of clean rock surrounding the pipe and connection. This was backfilled and the pipe was constructed through the creek. They 4" line was surrounded with sufficient rock bedding and the connections were lubed prior to connection. The amount of fall will be at 2% across the creek. THe contractor has backfilled as the pipe was layed.

Jeremy Young 03/05/2018 2:45 PM John DiMauro and myself checked the material avaliable on site. The boxes for 201, 203, 101 (partial) are on site. These boxes appear to meet the specifications. Box 101 only has one section avaliable at this time. There is no weir wall present in the upper portion of the box. I am going to check when the other sections arrive for weir wall construction. The RCP is on site and is correct for the job. They are AASHTO M170 and have both 24 and 30 inch sizes for line 100. The other storm lines are HDPE and have the connecting end covered and ready for installation.

This causes issue with the installed storm system line 200. The fix right now is to remove box 203 and approx 30' of pipe north and 20' south to make the adjustments. They will place the box and pipe back when the wall footing is complete. Once they have a final plan they will let us know.

We have asked and have not recieved an email regarding the change to the storm structure for Box 203. The contractor removed the structure and 20' section of pipe to the south and approximately 15' section of pipe to the north. I called Ed Moore this afternoon at 2:06 pm and left a message requesting a complete redesign of the retaining wall. Have not heard back from the contractor.

This causes issue with the installed storm system line 200. The fix right now is to remove box 203 and approx 30' of pipe north and 20' south to make the adjustments. They will place the box and pipe back when the wall footing is complete. Once they have a final plan they will let us know.

The contractor removed the structure and 20' section of pipe to the south and approximately 15' section of pipe to the north. I called Ed Moore this afternoon at 2:06 pm and left a message requesting a complete redesign of the retaining wall. Have not heard back from the contractor.

The contractor has been informed of the request and we are waiting for the plans. The retaining wall design has called for the sub grade material to be the same all the way across the wall length. The sub grade had to be over excavated to make it correct. The contractor has added AB3 into the over dig and we have requested information about the integrity of the process.

Inflow section has concrete poured just below the hole in the weir wall. This will allow them to finish when the metal plate arrives for the section. The toe wall for 100 was poured to specs for the tow wall.

Jerry Smith called me because he drove by and saw ashphalt trucks there and working in the rain. I spoke with Dan and we let them continue working as it had stopped raining. we also had an issue with the proper PG in the ashphalt, they had that issue resolved and are now installing the proper materials. The asphalt surface has been installed for the parking lot and the private road. This wraps up all the asphalt for this project. Brice Lawson 07/26/2018 9:13 AM A section of public sidewalk has been formed, the rock has been installed and compacted. The subgrade and compacted rock appeared to be stable. The sidewalk forms were set up with a cross slope less than 2%. The section of formed sidewalk begins 30' northwest of the driveway approach and extends approx. 170' to the northwest. The contractor plans to install 1" deep contraction joints on 5' centers, with a broom finish, apply cure and submit a copy of the concrete tickets.

She let me know that she talked with the engineer and that the sidewalk should be installed according to the approved plans. I spoke with Dan Harper about the slope of the sidewalk that will be installed in the sidewalk easement outside the ROW. He plans to go to the site to look at the site conditions and get back with me.

Brice Lawson 07/27/2018 3:54 PM I met with Dan Foster to discuss the placement of the proposed public sidewalk. He plans to extend the easement to the south and lay the sidewalk in the easement at a 5% or less running slope. He plans to submit a revised drawing to Sue Pyles. The sidewalk that was formed up just west of the drive approach was being installed. The contractor used 4K KCMMB concrete. They were installing contraction joints.

Jeremy Young 03/08/2018 9:55 AM Contractor added inverts and collars to flared end section. Also added to Box 202, 203, 204. They were poured with KCMMB 4K concrete. The inverts were rubbed and filled to the flow line. The collars were poured in the same KCMMB 4K interior and exterior. The box 201 and the toe wall for the flared end section will be completed at a later date.

The flared end section toe wall was al least 8" wide and 24" deep. The box inverts were all poured and collars also. They were poured to the flow line and KCMMB 4K mix was used.

The grading for the road and parking lot has been going on. I spoke with Ed about the set of plans that we have , the plans show an option of not putting down MO-type 5 if they use 9" of portland tilled in the dirt. Our specs call for 6" of Mo-type 5 and 7.5 of base asphalt and 2 of surface.
