Brice Lawson 07/23/2018 3:45 PM The driveway approach forms are set. It appears that 6" of concrete can be installed in the approach. I stayed and waited for them to get the compactor and compact the rock. I made Roy aware of the items that need to be completed listed on the detail that need to be completed prior to scheduling a pre-pour inspection. I also informed Roy that we will be using a 2' level for the post-pour inspection of the cross slope of the sidewalk.

A concrete ticket was submitted to me showing Dupuis as the supplier. -The load ticket that was submitted shows 20 gallons of water was added on site. -Dupuis does not have an approved KCMMB mix design at this time Make proper corrections to the sidewalk using an approved 4K KCMMB concrete mix. I informed Roy of the corrections today over the phone.

The concrete that was used is not an approved mix design. -The load ticket that was submitted shows 20 gallons of water was added on site. The ticket does not show the amount of trim water. -Dupuis does not have an approved KCMMB mix design at this time Make proper corrections to the driveway approach using an approved 4K KCMMB concrete mix. I informed Roy of the corrections today over the phone. I also asked him to schedule a pre-pour inspection when they are ready.
