STREETS. For SE Oldham Pkwy, the language used below the street name label needs revision depending on the intended timing for the recording of the subject minor plat. Use one of the following two language options: 1) If the intent is to record the subject minor plat PRIOR to the dedication of SE Oldham Pkwy by separate document, then add the following language beneath the SE Oldham Pkwy street name label: "R/W to be dedicated by separate document". 2) If the intent is to record the subject minor plat AFTER the dedication of SE Oldham Pkwy by separate document, then add the following language beneath the SE Oldham Pkwy street name label: "R/W dedicated by Doc # __________." Fill in the recorded document information in the blank prior to submitting the minor plat to the City for signatures.

APPLICATION. Submit a completed and signed minor plat application and Ownership Affidavit.

STREETS. 1) Label the ROW widths for US 50 Hwy, M-291 Hwy and SE Oldham Pkwy. 2) Add a street name label for SE Oldham Pkwy and provide a blank label for the document number that will dedicate said SE Oldham Pkwy ROW by separate document. The recording information will then be provided prior to the recording of this minor plat. If the minor plat is intended to be recorded prior to the dedication of the ROW, then label the SE Oldham Pkwy street name and include stating "ROW to be dedicated by separate document".

EASEMENTS. 1) An existing "20' Esmt. to Lee's Summit" is labeled along the north property line. The leaders for said label point to two (2) dashed easement boundary lines that scale to a width of 14'-7", not 20' as per the label. Revise the note, the leaders and/or the easement lines as necessary. 2) An existing "10' Missouri Public Service Esmt." is labeled along the north property line, near the northeast property corner. The left leaders points to a dashed easement boundary line that scales to a width of 5', not 10' as per the label. Revise the note, the leaders and/or the easement lines as necessary. 3) The minor plat includes a Sanitary Sewer Easement dedication paragraph, but it does not appear that any new sanitary sewer easement is being dedicated as part of the subject minor plat. Remove the sanitary sewer easement dedication paragraph if no such easement is being dedication via the subject plat.

SIDEWALKS. Show and label the location and width of the segment of public sidewalk to be constructed along SE Oldham Pkwy.

ADDRESS. Label the lot with the following assigned address: 10 SE Oldham Pkwy.
