INSPECTION REPORT RESULTS. The Maintenance and Condition Assessment Report performed by Tower Engineering Professionals, Inc., revealed a number of maintenance/repair items that needs to be addressed. Provide an action plan and timetable to address the identified maintenance/repair items. Documentation shall be provided indicating that the outstanding items have been properly addressed prior to approval of the final development plan for the co-locate.

PROOF OF INSURANCE. Provide proof of general liability insurance for claims from injury or death and property damage in an amount approved by the City, but not less than $2,000,000.00 per occurrence for personal injury and $2,000,000.00 per occurrence for property damage with the City listed and endorsed as an additional insured. The policy shall also include an endorsed waiver of subrogation against the City.
