Matt Munger 12/11/2017 11:54 AM During my visit I walked along the newly installed silt fence to see that it was installed properly. I observed the fence trenched into the ground and staked. TJ had also extended the fence as an added step to ensure silt does not leave the site.

I observed 6" and 8" that was marked SDR26 with a date of September 7, 2017. I also observed sewer manholes that had the black weather proofing coating.

Matt Munger 12/13/2017 11:00 AM Tom C. and I went out on site to check on the progress. I observed workers stripping ground and silt fence was still intact. TJ had brought up that they found two ponds on site and he was having a company come out and do compaction tests so they can address the issue. One of the ponds is where the public roadway is going to be put in. TJ stated the plan was to try and start Friday with the sanitary sewer but due to the pond issue that may get pushed back.

Matt Munger 12/15/2017 1:20 PM Bob Burns and I stopped by to check on the progress. Constuction entrance was in place off of Cheddington. MH A-1 and the sewer pipe was installed to MH A-2 prior to our arrival. KC Testing Lab was on site to perfrom the compaction tests. I observed a minimum of 6" of gravel under MH A-2. When we arrived half of the line from MH A-1 to A-2 had been backfilled. The half closest to MH A-2 that wasn't backfilled had clean gravel above and below the pipe (minumum of 6" above and below).

I observed and adequate amount of clean gravel above and below the sewer pipe. KC Test Lab was on site and the workers were using a vibratory sheepfoot roller over the dirt that had been backfilled from MH A-1 to MH A-2. I observed the workers using a laser to align the pipe. The backfill dirt piles around the site did not have any large rock or debris in them. I observed the workers lowering the pipe into the trench in a manner that would not damage the pipe that was being installed. When the workers were installing the pipe they used pipe lubricant and would seat each joint to ensure a tight seal. I asked Daniel length of pipe they used between the first two Wyes on the private main and he stated 5' of pipe was between the Wyes. I observed them install the 3rd Wye, they beveled the egdes of the pipe that they cut and set the Wye between 30-45 degrees and chocked in rock under the Wye to support it.

I observed the workers set MH B-2. I observed at least 6" of gravel in place before they set the MH B-2 base. They wrapped the joints between the base and barrel section. I observed Justin backfill and compact in lifts around MH B-1 to approximately Sta. 4+50. I spoke with the Sam from KC Testing and he stated he was doing compaction tests every 75ft. I asked him to email me the reports for what he had done so far.

I reminded Justin about using Utility tape 18-24" below final grade for the Public Sewer Main that was installed. We also discussed maintaining a minimum clearance of 18" from the flanges for the Backflow Vault when they start on they get to that point.

Matt Munger 12/20/2017 9:38 AM Upon my arrival I observed an adequate amount of gravel over the sewer pipe that had been laid from MH B-2 to B-3. There was an adequate amount of clean aggregate under MH B-3 as well as the black waterproofing coating on the exterior of the base. I had noticed the stakes were close together and reminded Daniel that there needs to be 4ft of visible pipe between the manholes, wyes and fittings. When the crew had lowered the section of pipe into the trench I had Daniel put his tape measure on the pipe and the section of pipe between the two Wye's was not long enough. Daniel had another pipe cut and put it back together so 4ft of visible pipe would be between fittings. I showed Daniel the LS Detail for Sanitary Sewer service stubs to show him where the trench check should be put and asked that he keep the ticket for the Flowable Backfill. Daniel had asked if they could use concrete and I informed again about using Flowable Backfill and showed him in Section 3500 in LS D&C on page 4 the requirements for Flowable Backfill. I observed the correct SDR-26 Sanitary Sewer pipe and looked over to see if there were any defects that I could see as well as seeing SDR-26 marked on the Wye's that were laying out. I also am sending the link to Daniel to the City of Lee's Summit Design and Criteria Manual.

Matt Munger 12/13/2017 11:00 AM Tom C. and I went out on site to check on the progress. I observed workers stripping ground and silt fence was still intact. TJ had brought up that they found two ponds on site and he was having a company come out and do compaction tests so they can address the issue. One of the ponds is where the public roadway is going to be put in. TJ stated the plan was to try and start Friday with the sanitary sewer but due to the pond issue that may get pushed back.

Matt Munger 12/21/2017 2:10 PM Curb Inlet 1-2 had been installed prior to my arrival. I did not see the flared end section installed. The silt fence behind CI 1-2 was taken down from the work that was done to install the curb inlet. The silt fence will need to be put back in place.
