INSPECTION REPORT RESULTS. A copy of a contract for services with Water Tower Experts has been provided for the completion of certain repairs/improvements to the existing water tower that were identified in "A Maintenance and Condition Assessment Report" performed by Tower Engineering Professionals, Inc. Prior to the approval of the subject final development plan application, documentation shall be provided indicating that the outstanding maintenance/repair items identified in the tower inspection report have been completed.

INSPECTION REPORT RESULTS. A Maintenance and Condition Assessment Report performed by Tower Engineering Professionals, Inc., was recently submitted to the City related to a similar project. The report revealed a number of maintenance/repair items that need to be addressed. A copy of the inspection report is viewable by using the following link: https://devservices.cityofls.net/Planning/Status?planningId=6323 Provide an action plan and timetable to address the identified maintenance/repair items. Documentation shall be provided indicating that the outstanding items have been properly addressed prior to approval of the final development plan for the co-locate.

PROOF OF INSURANCE. Provide proof of general liability insurance for claims from injury or death and property damage in an amount approved by the City, but not less than $2,000,000.00 per occurrence for personal injury and $2,000,000.00 per occurrence for property damage with the City listed and endorsed as an additional insured. The policy shall also include an endorsed waiver of subrogation against the City.
