The inspection report provided called out several (5) correction items that are needed for the tower. Please provide an updated inspection report once the defences are addressed and corrected. Also, the tower image in the inspection report reflects a lattice tower. The tower at this location is a monopole, please clarify.

The proof of insurance doesn't meet the requirements stated below. Only $1,000,000 was listed for per occurrence for personal injury and no amount was listed for per occurrence for property damage. It was also noted the insurance form didn't reference this tower/tower site. Please clarify. PROOF OF INSURANCE. Provide proof of general liability insurance for claims from injury or death and property damage in an amount approved by the City, but not less than $2,000,000.00 per occurrence for personal injury and $2,000,000.00 per occurrence for property damage with the City listed and endorsed as an additional insured. The policy shall also include an endorsed waiver of subrogation against the City.
