On Tuesday, 4-14-2020, I completed a inspection that had failed from Water Services. A video was taken to view the new sanitary sewer line and the inspection failed. Debris was found in the line and needed to be washed out. Also grouting needed to be completed on all (4) MH and lift ring. In was determined. None of the MH were exceptable and (1) had not been done at all. The owner was notified the work had not passed and he needed to get some else to jet the system and recover the debris.

DECEMBER 3, 2019 Inspector arrived onsite to meet contractors pouring curbs. Contractor asked how many cylinders HE was suppose to make for compression testing on the curbs. Inspector notified contractor that HE was not supposed to make any. Inspector also said to contractors that a 3rd party testing company was suppose to make the cylinders as well as test for slump and air content and cylinders, due to weather, were suppose to be left onsite until they reached 3500 PSI. Inspector then called Dusty Goppert and told him that he needed a 3rd party testing lab to test and make cylinders. Dusty informed Inspector that the job would be moved to December 4, 2019 and all parties would be on site to follow city procedures.

The strorm outlet structure was installed incorrectly, there was no fall going into the pond and the water was going back into the pipe the wrong way. This problem has now been corrected and the water is flowing into the pond and they also installed rip rap at the end of the outlet structure. This has been accepted.
