Provide proof of general liability insurance for claims from injury or death and property damage in an amount not less than $2,000,000 per occurrence for personal injury and $2,000,000 per occurrence for property damage with the City listed as an additional insured. The transmittal letter indicated that proof of insurance was provided with the application submittal, but no such copy was received by the City.
